We do not accept most Health Maintenance Organizations (HMOs). Standard Medi-Cal is not accepted as a primary insurance in our clinics. In the event that your insurance plan has not contracted for clinic based primary care or specialty services with Eisenhower Health, you will be responsible for your bill and may receive partial payment from your insurer. While every effort is made to ensure this list is current, it is always best to call your carrier to confirm both in and out of network benefits prior to making an appointment. The list of health plans at Eisenhower Health continues to grow. Insurance Accepted for Primary Care & Specialty ClinicsĮisenhower specialty clinics, will ultimately accept most insurance plans. We are preferred providers for many insurance carriers however, it is best if you contact your carrier directly regarding your specific benefits. You may also call the Billing Customer Service line at 80. Use "Billing Question" as the subject for the message. If you have a question about billing, you can send a Customer Service message from your M圜hart account. If you wish to pay by cash or check, please send all payments to our offices. 
Credit card payments can be made electronically through the Pay my Bill form.We take cash, checks, VISA, MasterCard, Discover and American Express. Co-pays and account balances are due at the time of service.
If we are not a preferred provider, the percentage of the bill you will be personally responsible for will be most likely higher. However, it may be necessary to schedule you with another provider and/or another location. We will make every attempt to schedule you to see your preferred Eisenhower Primary Care PCP. Eisenhower Health will ultimately be preferred providers for most insurance plans.
